I authorize Clue Entertainment Centre Inc. the right and permission to copyright and/or publish, reproduce or otherwise use my name, voice, and likeness in video, photographs, written materials, and audio-visual recordings. I acknowledge and understand these materials about or of me may be used for both commercial and/or non-commercial purposes.

Cancelation and Refund Policy: If you need to cancel your class, we respectfully request at least 1 week notice prior to the first session. The full term tuition will be non-refundable after the first session. I agree to this policy.
